I'd go for the earlier idea of swapping spots, however I'd also set a limit of the number of times a player may be challenged per week without allowing him to refuse, say, 3?
Above and beyond that figure, the challenged player (presumably of higher rank) has the option to refuse the challenge no-questions asked. Assuming wins,losses,draws and refusals are all recorded on the ladder ifa player keeps refusing he may find his opwn challenges refused etc.
The downside is that you'd end up witha very odd looking ladder, the top player perhaps having won few games and lost/refused a great many, still it's an intersting idea and certainly worthy of further development.
